Why and when is a model release necessary ?
Model Release Form
As a general rule photographers should possess a signed model release from all models present at a photo shoot.   Even if one had no intent to use, sell, or post images of a model without their written consent, a signed relase allows such use to occur once permission is granted.

The following link provides an example of a model release form.
